I, the undersigned participant, hereby voluntarily agree to participate in activities organized and conducted by the Table Tennis Association Club (hereafter referred to as “the Club”). I understand that engaging in table tennis activities involves certain inherent risks, and I agree to release, discharge, and hold harmless the Club and its organizers, agents, representatives, volunteers, and all associated individuals from any liability, claims, demands, and causes of action arising out of or related to my participation in Club activities.
Assumption of Risk: I acknowledge that table tennis activities involve physical exertion and the potential for injury. I understand and appreciate the risks involved, including but not limited to the risk of collisions, falls, equipment failure, and other potential hazards associated with physical activities. I voluntarily assume all such risks and waive any claim against the Club and its representatives for any injury, loss, or damage that I may incur during participation.
Medical Authorization: In the event of an injury or medical emergency during my participation in Club activities, I authorize the Club and its representatives to seek medical attention on my behalf. I also agree to be responsible for any medical expenses incurred as a result of any injury sustained during Club activities.
Release and Waiver: I hereby release, discharge, and agree not to sue the Club, its organizers, agents, representatives, and volunteers for any claims, demands, damages, actions, causes of action, or suits, whether in law or in equity, arising out of or resulting from my participation in Club activities.
Photography and Publicity: I grant the Club permission to use any photographs, videos, or other recordings of me taken during Club activities for promotional and educational purposes.
Severability: I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the state in which Club activities take place. If any provision of this waiver is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
By signing below, I acknowledge that I have read and understand the terms of this waiver and voluntarily agree to its provisions.
